Non-Member Registration

Hello! In order schedule a one-time non-member appointment at Peach Blossom Pediatrics, there are a few steps to follow:

  1. Click the “REGISTER NOW” button below

  2. You will be redirected to the SigmaMD Enrollment site

    • First enter your own information and click the radio button on the bottom where it says “I am enrolling others, but not myself”

    • Click “save,” then click “add” to enter each child’s information

    • Then you will enter your payment information and attest to having read the Fee For Service Agreement. You will be charged a $50 “enrollment fee,” which serves as a deposit and holds a spot for you. This will be applied toward the total bill for the visit, with the balance being charged at the time of the service.

    • You will receive an email with a registration code for the SigmaMD app - be sure to download the app and register, as this is how you’ll schedule the actual appointment.

    • SigmaMD will prompt you to complete their standard intake form - you do not need to do this. Instead, move on to step 3 below.

  3. After your SigmaMD enrollment is completed, please complete an Non-Member Intake Form for each child you are registering. You will receive the link to do this through the SigmaMD app and email.

  4. Schedule the appointment on a day/time that is convenient for you via the SigmaMD app.
